I highly recommend checking out dspam (mysql backend) + clamav as a
solution. I've switched to it from a handful of other things, and it
blows everything else out of the water. It's very database intensive, so
see the MySQL performance tweaks on their site, but the results are
worth it. Plus, it has a nice Debian package, integrates well with Exim,
and comes with a builtin web interface for users to configure their
scanning and do training.
