I get this all the time and it drives me crazy. At least once a day.
I don't use midnight commander. I've never actually checked what the
server side is doing. I'm in konsole and usually send that tab a HUP.

For me it only seems to happen if a session has been running for a
long time (If I log it and out within, say 20 minutes, and doesn't
happen). It happens when I ssh to a variety of different ubuntu
servers from my ubuntu desktop. I don't recall this happening before
when I had a debian desktop and a bunch of debian servers.
